Welcome to the First organized in 1989, the Illinois Rural Health Association (IRHA) is a collaborative association comprised of hospitals, Federally Qualified Health Centers, Rural Health Clinics, public health departments, individual clinicians, educational institutions, students and elected officials, whose mission is to strengthen health systems for rural residents and communities through advocacy, education, networking and leadership. Community Behavioral Healthcare Association (click here for additonal resources) RHTP GRANTS Due FRIDAY, OCT 23 at 5 pm Eligible*: Behavioral Health Clinics, Certified Behavioral Health Clinics, & Substance Use Treatment Facilities (*but other entities can partner with them) Community Care Infrastructure/Integrated Care - Access the NOFO Here. Funded projects will support the development, expansion, or strengthening of integrated, team-based models of care that bring together behavioral health, primary care, and community partners. Integrated care may include multidisciplinary care teams, shared workflows, coordinated care planning, referral management, care transitions, information sharing, health information technology, and formal partnerships that improve communication, coordination, and whole-person care delivery. Overcoming Geographic Barriers to Care: Mobile Healthcare Innovation, Including Mobile Crisis Response, for Rural Illinois, - Access the NOFO Here.Funded projects will support the development, deployment, expansion, modernization, or long-term sustainability of mobile health models that enable direct behavioral health service delivery in rural Illinois.
